Observation

110-300-0165(2)(a) - Licensor observed the children toddler had access to material that posed a choking hazard. Licensor observed child open toy with small toxic pellets inside and attempt to ingest the items under providers supervision. 110-300-0165(2)(c) - Licensor observed three cords from portable heaters were accessible to children. These were lose cords not plugged in to an outlet and are able to form a loop around a child's neck. 110-300-0165(3)(a) - Licensor observed nails protruding from broken plastic material laying on the outdoor play area. Licensor also observed the metal bar on a bicycle to be broken exposing a sharp edge. 110-300-0165(3)(b) – Licensor observed three portable heaters accessible to children posing a burn hazard. 110-300-0165(3)(c) – Licensor observed the bifold closet doors for the laundry equipment was accessible to children with no childproof lock and poses a pinching hazard. 110-300-0165(3)(d) – Licensor observed that the washer and dryer were accessible to children with no childproof locks, imposing an entrapment hazard. 110-300-0165(3)(f) – Licensor observed that a shelf in the back child care area was not securely anchored to the wall. Licensor was able to easily move the shelf back and forth showing a tipping/falling hazard. 110-300-0165(4)(a) – Upon arrival licensor observed that the temperature in the back child care room was below 68 degrees Fahrenheit. 110-300-0165(4)(c) – Licensor observed lightbulbs located next to the kitchen/backdoor area to not be shatter proof or have a shatter-resistant cover. 110-300-0165(4)(e) – Licensor measure water temperature at handwashing sink in the bathroom to be 123 degree Fahrenheit. 110-300-0165(5)(a) – Licensor observed outlets in the licensed space area to not be tamper resistant. 110-300-0165(5)(b) – Licensor observed the outlet near the sink located in the bathroom to be GFCI but not tamper resistant.
